Vasectomy vs. IUD or the Pill: Why More Couples Over 35 Choose Male Sterilisation

‍

She’s been on the pill since her twenties. Two kids, three house moves and a career later, she still thinks about it every morning. Or she has an IUD she’d rather not have, but hey, it works.

You both know the family is complete. So why is she still carrying the load?

More and more men over 35 are asking themselves the same question. And they reach the same conclusion: time for me to take over.

‍

Twenty years of contraception is enough

In most relationships, contraception is her job. Remember it every day, renew the prescription every year, book a new appointment every few years. And all that time she’s putting synthetic hormones into her body. For years. Without anyone asking whether it’s still necessary.

  • Blood clots. The combined pill makes blood clot more easily. Pill users have roughly 2.5 to 6 times the risk of thrombosis or pulmonary embolism compared with women not on it. The risk rises with age, and smoking, excess weight and migraine make it worse. At 20 it was a footnote. At 40 it’s a real risk.
  • Breast cancer. In a Danish study of 1.8 million women, risk rose 38% after more than ten years of hormonal contraception, the hormonal IUD included. In absolute terms it stays small, but after twenty years on it, she’s exactly that woman.
  • Mood. Users of hormonal contraception are more often prescribed antidepressants. Many women only realise after stopping how they felt all those years.
  • One slip is enough. A missed pill, a stomach bug, a course of antibiotics: in real life about 1 in 11 pill users gets pregnant within a year. When the family is already complete, every late period feels like an alarm.
  • The IUD. Insertion can be painful, the copper type makes periods heavier, and in rare cases the IUD shifts or perforates the womb.

The IUD served her well. But she doesn’t have to do this for another twenty years.

What you think happens (and what really happens)

“I’ll be less of a man.” No. Your testicles keep making testosterone. Sex drive, erections and orgasms stay the same. The only difference is that there’s no sperm in the ejaculate, which is a tiny part of the volume. Nobody notices. You won’t either.

“Isn’t that castration?” No. Castration removes the testicles. In a vasectomy everything stays in place. Only the two tubes that carry sperm are cut.

“That must hurt.” It’s done under local anaesthetic. Afterwards you have a few days of swelling and bruising.

‍

What you both get

  • Reliable. If the check after three months shows no sperm, the chance of pregnancy is very small, smaller than the pill in everyday life.
  • Hormone-free. She can stop the pill or hormonal IUD. Your hormones don’t change.
  • No more hassle. No counting pills, no prescriptions, no debate about who remembered.
  • One and done. A single minor procedure instead of another twenty years of daily upkeep.

Couples often mention one more benefit: the tension disappears. No “just to be safe”, no panic when a period is a day late.

How it works at De Herenkliniek

1. The consultation. You visit us in Hilversum, Rotterdam or Amsterdam. We talk through your situation, your questions and why you’re considering this. We only continue if the decision is well thought through.

2. The procedure. We use the classic technique under local anaesthetic. Afterwards you rest for a few days.

3. The check. After about three months you hand in a semen sample. Only when it shows no sperm can you stop other contraception. Until then, you stay protected.

Honestly

A vasectomy is meant to be permanent. Reversal is possible but doesn’t always work and isn’t reimbursed. There are risks too: bruising or a haematoma, an infection and, in a small proportion of men, long-lasting scrotal pain. We go through these beforehand so you make a choice you won’t regret.
